Newcomb Senior Center

The Senior Citizen Center is located next to the Chapter House and they serve congregate meals and they do home delivery. The Senior Citizen Center also provides transportation.

Mission Statement: Division of Aging and Long-Term Care Support shall ensure that all elders living on the Navajo Nation receive all available benefits and services to which they are entitled and that these services are provided in a manner which preserves their dignity, self-respect and cultural identity.
Purpose
The purpose of the Division of Aging and Long-Term Care Support (DALTCS) is to provide meals transportation, health, personal, social, recreational, and referral support and services to eligible Navajo individuals in coordination with tribal and non-tribal agencies and entities. DALTCS has been providing nutrition and non-medical home and community based services on the Navajo Nation since 1974. These service are provided to older adults, their spouses, individual with disabilities living with elders, age 60 and over, living     on or off the Navajo Nation.

Congregate Meals

Purpose Statement
This service helps to increase the nutrient intake of participates to prevent or reduce the risk of chronic diseases, preserve and promote health, and improve nutritious status.
Service description
Taxonomy definition—A  service that provides for a nutritious meal containing at least 1/3 of the Recommended Dietary Allowance for an individual in a congregate setting.
1. Provide for meal planning, preparation and service.
2. Provide staff training, nutrition education and social interaction.
3. Link older adults with community-based services and provide resources that give participates choices for physical and health interventions, where available.

Home Delivered Meals

 Purpose Statement
The service helps increase the nutrient intake of older adults at nutrition risk and allow them to remain independent in their homes.
Service Description
Taxonomy definition—A service that provides for a nutritious meal containing a least 1/3 of the Recommended Dietary Allowance for an individual, delivered to his/her place of residence.

General Transportation

Purpose Statement

The service helps to assist older individuals and individuals with disabilities to maintain their independence and avoid costly and unwanted placement in a care facility by providing access to services.

Service Description

Taxonomy Definition—A service that provides or assist in obtaining various types of transportation for specific needs.

1. The service includes the arrangement/provision of transportation services which may include the use of a van.

2. Senior center participants may be transported from their place of residence to the center and returned to their residence; they may be transported to appointments or other community services such as shopping and Post Office.

3. Services may include the transport of groups to activities such as recreational, educational or community events.

4. Eligibility Requirements—Newcomb Senior Center shall provide services to individuals who meet the eligibility requirements described in Chapter 3000, Section 3100 and 3600 of the DAAS Policy and Procedure Manual, as may be amended.

1. Home delivered nutrition services provide older adults, in their home or place of residence, with nutritious meals that meet 1/3 of the Dietary Reference Intakes. 
2. Home delivered nutrition services provide resources and options, when available, that allow older adults to remain independent in their homes and communities.

3. A “wellness check” is conducted at the time of the meal delivery to evaluate the general health and well being of the individual.

4. The service also provides for the opportunity for socialization.

5. Eligibility Requirements—The Newcomb Senior Center shall provide services to individuals that meet the eligibility requirements described in Chapter 3000, Section 3100 of the Division of Aging and Adult Services (DAAS) Policy and Procedure Manual, as may be amended.
